you receive at Assured Care Network Clinics; discounts range from 25%
to 75%, discounts are based on income and family size; includes doctor
visits, tests, hospitalization, dental care, prescriptions, chiropractic.
To be eligible, you must live in Hennepin County, have no health insurance
or meet Assured Care guidelines, not be eligible for Medical Assistance
or Medicare, not have assets above Assured Care guidelines, and have
a household income within certain guidelines (example: 98/99 guidelines
for single person/no children must be less than $1342 monthly income). |
