- 1)________ CLASS Prospective Student Information Form (.pdf file, should open with most word processing software)
2)________ Copies Of Your Diagnostic/Medical Evaluation(s) (completed within the past 3 years) See the CLASS Documentation Policy for more information.

3)________ Personal interview with one of the CLASS Specialists. Parents are welcome at interviews. Telephone interviews are possible if distance is a significant issue. Call CLASS at 612-330-1053 to schedule. Interviews usually take one hour and can be incorporated into a campus visit through the Augsburg Admissions office.
