Tax Forms

You must give the Enrollment Center copies of the following before financial aid may be disbursed:

Dependent Students: You must send a signed copy of the first two pages of your own 2006 1040 form, as well as a signed copy of the first two pages of your parent's 1040. If your tax forms were self-prepared or e-filed, you must sign each page of the tax form.

Independent Students: You must send a copy of the first two pages of your own 2006 1040 form only. If your tax forms were self-prepared or e-filed, you must sign each page of the tax form.
If you are married and filed separately, you must send a signed copy of the first two pages of your own 2006 1040 form as well as a signed copy of 2006 tax forms for your spouse.

Can’t find your tax return? Call the IRS at (800) 829-1040 to order a tax transcript.

Be sure to include your name and student ID on every form you send in! The Enrollment Center must receive these documents before financial aid can be disbursed.
