Student Teaching Evaluation Forms
The following forms are used to report evaluations of student teaching performance. A copy of these forms are also printed in the Augsburg College Student Teaching Handbook. Click on the bold hyperlinks (below) to access the HTML versions of these forms. (If you would prefer a word processed version, a copy of each form is accessible by clicking on the hyperlink below each description.) Any of these items may be printed and used in lieu of the print versions.
Observation Form: The Observation Form is used by the Augsburg supervisor and secondary content-area specialists to record their observations and evaluations when visiting a student teacher in the field. The criteria used in this form are explained in detail in the Augsburg College Student Teaching Handbook.
Student Teaching Progress Report: This form is utilized when a teacher candidate is not meeting the established standards for student teaching. Following observation by the College supervisor and/or evaluation by the host teacher, the standards checked and described on this form are matters of concern that need to be addressed by the candidate in order to successfully complete student teaching.

Mid-Term Evaluation Form: The mid-term evaluation form is completed by the K-12 cooperating teacher and is shared with the student teacher and his or her college student teaching supervisor. It is formal notice of the student teacher's progress on the criteria to be used in the final evaluation at the end of student teaching. These criteria are explained in detail in the Augsburg College Student Teaching Handbook.

Final Evaluation Form: The final evaluation form is completed by the K-12 cooperating teacher and is shared with the student teacher and his or her college student teaching supervisor. A copy of the completed evaluation is also placed in the student teacher's permanent licensure file at the college. It is formal documentation of the student teacher's progress on the criteria discussed in the Augsburg College Student Teaching Handbook.
Student Teaching Program Evaluation Form: This form is used by the K-12 cooperating teacher to evaluate the student teaching experience--the contact with the Education Department office, supervising professor, etc. It is used within the Education Department to improve future K-12 cooperating teacher's experience with Augsburg College.
