Confirmation Form   

Please confirm with us that you acknowledge you have been accepted to this program, and whether or not you intend to participate.  Programs sometimes have waiting lists, and we will need to know as soon as possible if you will be attending, or your space will be available for another applicant.  A $500.00 Deposit  is due within three weeks after acceptance (i.e. when you receive your acceptance letter). This non-refundable deposit will hold your spot in the program, and be applied to your comprehensive program fees. (Please call our office immediately if the funds are not available during this time frame.)


Additional Biographical Information

We collect this information for a variety of purposes: for enrolling you in insurance, acquiring visas, and creating an account at Augsburg College so you can be registered for classes. * Please note that the starred items are required.


Registration and Payment Procedures for Non-Augsburg Students:   

After discussing these options with your study abroad coordinator, complete this form and return it to our office.


Participant Information form 

It is important for us to know how to contact you during the upcoming months, particularly during the summer and breaks, so please return this form as soon as possible.


Medical Report form   
and Physician’s Evaluation 

Be honest and complete with the information you provide. This is very important in ensuring that you are healthy and able to fully participate in the program. The semester is very rigorous, both physically and emotionally, so the better you prepare yourself, the more rewarding your experience will be. Please refer to our general Health Information, and it will also be helpful to review on-line sources such as The Centers for Disease Control website. Your good health, or lack of it, can be significant in what kind of experience you have during the semester. Keep this information in mind when preparing for the program.


Registration Form   

Please review the Course Registration Information, course syllabi and general information about registering for your program (see link below),  and consult with your academic advisor before filling out this form. Once you have completed the Registration Form, make a copy for yourself and send the original to the Minneapolis Office.

Carefully read this form, sign it, and return it to our office.


Passport Number:

If you did not include this information on the “Immigration Information” of your program application, please call or e-mail the Center with the number.


Passport:

If you do not already have a passport, you should apply for one as soon as possible. Don’t put this off! It is very important that you have your passport well in advance of the program (especially if you are a part of the Namibia program and will be applying for a visa).  If you have one, make sure it is valid for six months past the end date of your program.  If it isn't, please apply for a new one.  You can apply for one either at a passport office, county government building, or a major post office. You will need two passport photos, a certified copy of your birth certificate (with a raised seal), and a check or money order for the processing fee.  Birth certificates are available from the clerk of court in the county in which you were born, in case you need to get one. In fact, get two copies so you can bring one with you (just in case you lose your passport and need to apply for a replacement).  For more information see the U.S. State Department webpage at: http://travel.state.gov/passport/passport_1738.html


Transcript Request form 

Grades are not automatically sent to your home school at the end of the semester. Federal law requires that you must submit your request for a transcript in writing. Please return this form to the Center for Global Education. Make sure you verify with your home school where the transcript should be sent to guarantee transfer of credit.  You do not need to provide your ID number. Your Augsburg ID number will be provided by the semester Programs Abroad Office. You are provided one transcript free of charge, so you do not need to send any money for one transcript.  However, any additional requests will be charged a $6 fee.
