All written
documents submitted to the IRB must display college-level writing
proficiency. Please attend carefully to grammar, spelling, and
punctuation. Poorly written essays and inadequately organized
applications will be rejected. It is strongly recommended that you print
and submit your application double-sided to save paper. Do not submit
the instructions with your application--only the actual application
needs to be submitted to the committee. Please submit your complete
mailing address and e-mail so that the IRB can communicate with you.
You may download an
electronic version of the IRB Application Form from this web
site. There are two options:
1. You may download the
IRB Application Form as a word processing document. You must
have access to the appropriate word processing software in
order to use these documents. Microsoft Word is preferred.
2. You may also download a Portable
Document Format (PDF) version of the IRB application. This
form can be viewed and printed from any computer which is
equipped with Adobe Acrobat Reader, available from
Adobe
Systems, Inc. PDF
documents cannot be edited, so if you wish to type
your responses on a computer, you should choose option 1
(above).

Forms:
Application for Extension of
IRB Approval: This form is to be completed by the
researcher (and his/her advisor, if appropriate) in order to
receive an extension if the project is delayed beyond the dates
specified in the original proposal approved by the IRB.
Brief Application for Continuation of Classroom Research
Projects: A form for Augsburg faculty and staff---for
expedited or exempt reviews only.
Secondary Data Review: A form for studies using private
secondary data, not published or public data.
Sample Consent Form: Use this form as a template to
prepare your consent form. All
of the specific elements of this sample form represent crucial pieces
in establishing informed consent. Use all relevant elements of this
model form in your form.
Special Note: A helpful strategy for filling out word-processed forms is to double-click on the underlined areas, which
highlights the underlined area, and then start typing. This deletes the
underline and replaces it with your typed response. Similarly, where
large open areas are provided for a response, you should delete the
blank lines in order to use the space taken up by the blank carriage
returns for your response. If items move substantially or become
mis-alligned as a result of your deletion, you may have to add spaces
or carriage returns in order to move the the affected part back. In
general, please fill out the form in such a way as to preserve spacing
and pagination so the IRB readers can easily follow your application
text.

installed on your computer (e.g., Microsoft Word or Adobe

Guidelines for submission:
Members of the Augsburg community should bring 3 copies of an expedited
or exempt proposal to Oren 306. Eleven (11) copies of full review
proposals should be brought to
Memorial 111. An electronic copy of all proposals should be sent
to <noonan@augsburg.edu>.
In general, do not U.S.P.S. mail the proposals to us. It will greatly delay the process. Individuals in the
metropolitan Twin Cities area should not use the US mail since it slows
down the process.
