Semester Check-In Form

At the END of every semester, F-1 International Students are required to connect with the ISSS office about plans for the following semester. This information allows ISSS to review your F-1 files, and make important corrections to your SEVIS record if needed.  In addition, your check-in form response will help ensure that:

  • your immigration status is valid, including any change of address;
  • you have access to current and valid health insurance; and
  • you have an opportunity to share your goals for the coming year.

During COVID-19 this Immigration Check-In has two steps: 

  1. Completing the immigration Check-In form that gives answers to your plans and goals for the following semester.  Please submit your Fall Immigration check-In Form by April 21,2024.
  2. Please also fill in the Lewermark Insurance Enrollment Form.
